Patient is a lovely 26 year-old female who presented complaining of “hard implants”, scar-tissue in her breasts with deformity and pain. She had her primary breast augmentation procedure performed in Mexico City back in July 2012, and states she had “silicone gel” implants but did not know the brand or volume or plane of placement. She reports that she had drains postoperatively, and that she immediately had problems with her left breast that “took three weeks to close”. She states that she finally had to return to the US and noticed worsening hardening of her implants about 1 year ago. She then had a breast ultrasound performed at Kaiser that revealed “fluid buildup between her muscle and implant” and she was concerned. She also complains of unsightly inframammary scars and pain. She was so embarrassed by her breasts that she “won’t let her husband even touch them”. She thought she wanted to exchange to saline for “safety reasons”, however after counseling in the presence of her husband, they preferred to exchange to soft, silicone gel implants for a more natural look and feel. She wears about a D-cup bra but her implants are encapsulated high up and her breasts drooped over her implants giving a very odd shape. She desired to stay about a D-cup but much softer with a prettier, more natural shape. I placed her on Singulair peri-operatively to try and combat against recurrent capsular contracture.
